Application for New Rapid Rehousing Projects


This application has been released by Opening Doors of Fairfield County (ODFC)/CT-503 to solicit proposals for Rapid Rehousing (RRH) for youth 18-24, single adults and/or families to be administered by the CT Department of Housing (CT DOH).  Funds will be awarded by the Department of Housing and Urban Development (HUD) through the Notice of Funding Availability (NOFA) for the 2016 Funds in the Continuum of Care Program Competition, a national competition using the scoring criteria defined in the FY16 NOFA. The ODFC jurisdiction includes three sub-regions : Greater Bridgeport, Greater Norwalk and Stamford-Greenwich and the following cities and towns: Bridgeport, Brookfield, Darien, Easton, Fairfield, Greenwich, Monroe, Newtown, New Canaan, New Fairfield, Norwalk, Redding, Ridgefield, Shelton, Sherman, Stamford, Stratford, Trumbull, Weston, Westport and Wilton.

I. Project Description.

The total potential new funding for a bonus round of projects is based on the FY 2016 NOFA. ODFC will dedicate all new funding toward a Rapid Rehousing bonus project(s). ODFC is seeking proposals from eligible 501(c)(3) nonprofit organizations to provide culturally competent support services via housing relocation and stabilization services, as necessary, to assist young adult individuals, adult singles and families living in shelters or in places not meant for human habitation (for example, living on the street or in a car) to move as quickly as possible into permanent housing with a goal of achieving housing stability. Services will be provided within a specified sub-regional catchment area (as described above) and applicants from the three sub-regions within the CT 503 jurisdiction (Greater Bridgeport, Greater Norwalk and Stamford-Greenwich) are encouraged to apply.

Sub-grantee contracts will be assigned to at least 1 regional Sub-grantee for the entire ODFC region OR up to 3 Sub-grantees located within the region. The selection of the Sub-grantee(s) will be determined by the pool of applicants that respond to this RFP, but collaboration and capability to offer services in an integrated setting to a specified target population, illustrated by providers as applicants, is expected to be given additional points when scored. The project(s) will be delineated across target populations as follows:

1. A portion of the funding will be directed to youth, based on assessed vulnerability between the ages of 18 and 24

  • Target populations will be extracted from the universe of the Fairfield County Coordinated Access Network (FC-CAN) Housing Registry
  • All eligible clients to be assessed and prioritized via the use the ODFC common assessment tool, the TAY-VI-SPDAT (Transition Age Youth-Vulnerability Index-Service Prioritization Decision Assistance Tool), and scores must be within the appropriate range for the RRH level of care as determined by the FC-CAN

2. A portion of the funding will be directed to individuals and families with a head of household aged 25 or older

  • Eligible clients will score within the appropriate range for RRH level of care as determined by the FC-CAN
